Longest wait on appointments is the following day. You can fly round trip from Eugene or Medford for under $500. But to do that need to fly to Las Vegas or Los Angeles.

Normally you will otherwise find yourself flying to Seattle or Denver at added cost. Las Vegas has the lowest costs in the Western US. I frequently fly from San Jose Costa Rica to Ft Lauderdale for under $225 round trip. Even flown one way for $66 with Priority bording and Comfort Plus Seat. I am not a dentist or travel agent and not out for profit. Simply wanting to inform people of more affordable dental procedures and provide some added income for Costa Rican dentists. My own dentist has no more than two patients daily. Best dentist I ever had too. Now another item. As far as US immigration goes, someone left the back door wide open. While most nationalities must pre apply for a US Visa at a US Consulate or Embassy and be interviewed, that is not the case with Puerto Rico where almost anyone will be granted a 90 day visa upon arrival. Being a Territory of the US, that Visa stamp in a passport will be USA. And with that a person can easily travel to the US mainland. Also ferries regularily travel to Florida from Caribbean Islands such as Bermuda and Bimini and no passport required. Only a birth certificate and government photo ID. I really hit it good. I rent a 4235 sq ft/6 bedroom/5 bath/ 8 car garage villa here for $230 a month. My electric bill with an all electric kitchen is always less than $10. Water $3, 4G Internet $14 I haveno neighbors from other countries living within 20 kilometers and plan on keeping it that way. Where most North Americans live such as Escazu, a villa like mine would be renting for between $4000 and $6000 a month. While you can live well here for about $1000 a month, stick with renting and never buy. As to working, you are excluded from $100,400 annually in US Income Tax if you remain outside the US 330 days a year. And Costa Rica as well as other countries only tax on money made inside the country. Make it by Internet or phone and you have no tax liability here. Making money inside Costa Rica is nearly impossible. Very few skills are needed, least of all constructiobn. And to open a business first step is to deposit $200,000 into the Bank od Costa Rica, then hire two Costa Ricans for two years. If you fire them they still get the two years pay and you have to hire two more. You can see that woud be a bottomless pit. Everyone loves a two year paid vacation.
